Disability Outreach partners with organizations that are invested in creating healthy communities. We are committed to helping disabled individuals access the benefits they need, while helping maintain the fiscal health of Medicaid MCOs, governments, and community agencies.

Medicaid MCOs

Disability Outreach works with managed care organizations to transition disabled members to the SSI category of eligibility. By providing members with expert, empathetic advocates, MCOs are offsetting the financial impact of supporting disabled members, while offering an important social service that fosters positive relationships between the member and plan. While plans are capitated at a significantly higher rate, they also increase plan loyalty by providing members a free service that grants those members access to monthly income to meet their basic needs.


State and Local Governments

Disability Outreach partners with state and local governments to help the disabled population currently receiving public assistance benefits to access SSI benefits. By doing so, those who are found eligible for SSI will begin to receive a monthly federal benefit providing them with more financial stability, along with the continued health care they need. The increased SSI utilization helps a state use its own and federal resources appropriately.


Community Agencies

Disability Outreach works with community agencies to help disabled individuals access the Social Security benefits they need. Our expert, empathetic advocates assist the people you serve and help them access the benefits. If you are one of our partners, you can complete the online referral form, and we will begin working with the referred person to assess if they may be eligible for SSI.
